Please follow these steps:

• Please note that this tool is only available for patients who are registered/enrolled in family practice at Dr Tiab office

• Access our office online booking web site: https://portal.indivicare.ca/b/drtiaboffice

• Set a date range to search for available appointments (only one week from start date can be shown at a time)

• Select the date & time slot of your choice.

• Fill your demographic information as required.

• Scroll down to the bottom of the page and click submit…All done.

• You will receive a confirmation e-mail with your appointment.

• If there is any mismatch of the information with our data base, you will receive a massage saying “Unable to match patient record” at which point you should make corrections or contact our office for assistance.

• Please contact our office @ 289 837 4411 if you need urgent appointment when there is no available spot.
